Provision of our health services

Data collection in the provision of healthcare services at the practice

In connection with your treatment at our practice, we process general personal data and health data.

On the personal information forms (physical or digital form) for registration as a patient, you provide us with general personal data (namely name and contact details). You also provide information about treating physicians and your insurance. Some of this personal data (patient master data) can also be obtained directly from your insurance card or a database of your insurance.

In the course of providing the service, we process your health data. This includes information about your past or current health condition, your physiotherapy, therapy progress, as well as the use and billing of supplementary health services.

Health data (in the form of text, image, sound, or video) is collected or created by our therapists during examination or treatment at the practice. We also receive health data from physicians who refer you to us or have treated you previously.

Communication with You, Information, and Additional Services

Personal data in regard of communication

If you contact us via email or telephone, provide feedback or inquiries through the feedback form on the website, apply for a position, or register for physiotherapy, consultation, events, or courses, we will collect the information you provide to us.

This may include general personal data such as first name, last name, email address, and telephone number. If we offer online appointment booking, we will collect the personal data you provide to administer the appointment booking.

On our feedback form, you can provide information about the reason for your physiotherapy. The submission of feedback and the provision of health data in this context are always voluntary. If you apply for a position, we will collect your resume and additional information about your qualifications that you submit to us as part of your application dossier.

Appropriation and use of the website and online portal

Use for information purposes

You can visit our website to gather information about Z2PHYSIO and our range of services without disclosing your identity. Like with any connection to a web server, the server on which we host the website and online portal automatically logs and temporarily stores certain technical data. This includes the IP address and operating system of your device, the date and time of usage, and the type of browser used to access the website or online portal.

We offer you an online portal ("Patient Portal") to facilitate communication and administration with us. When you fill out a form or register as a user on the online portal, we collect the information you provide to us. The web analytics services we use collect and store user data using cookies. Before being transmitted to a server outside Switzerland/EU/EWR, the requesting IP addresses are shortened.
